Use this report to find LCDs that allow a particular service for a particular diagnosis.

Create a custom report by following the steps described below.

Step 1

Begin by entering a CPT/HCPCS code you would like to search on. In this step, you are required to enter search criteria into the gray box.

Step 2

Now enter an ICD-9 code you would like to search on. This is also a required search criteria. If you do not know the ICD-9 code, you can look it up using the "code lookup" link.

Step 3

Now enter any additional search criteria, if desired, to narrow down the scope of your report. For the Region, State, Contractor Type and Contractor criteria, the actual selection criteria are hidden from view until the appropriate radio button is selected. Note that by default no restrictions are placed on the report.
