Non-committee members must register via email or regular mail to Sean Dalenberg if they want to attend the meetings. Registrations must be received by 5pm, September 16th, 2005. Addresses are as follows: sean.dalenberg@cms.hhs.gov or Sean Dalenberg, Mailstop C5-08-27, 7500 Security Boulevard, Baltimore MD 21244. An agenda identifying subjects to be discussed has been posted on this website. Each registrant must provide the following information: Name, telephone number, email address (if applicable), company name and address. Because of staff and resource limitations, we cannot accept registration/comment by facsimile (FAX) transmissions. Registration for Panel Presentations -- See downloads section below. |
