What Your Client Needs to Apply
The following are sample questions that your clients may
need to answer if they apply for SCHIP. On some questions, your client
will have to check all the answers that apply based on his/her situation.
- Are you (or your spouse) pregnant?
- Are you currently receiving,
or are you eligible to receive any other benefits?
- Do you have a child who is not
covered by health insurance, including Medicaid?
- How many people make up your
family?
- What is your child’s citizenship
status?
