The Doctors' Office Quality (DOQ) project was designed to develop and test a comprehensive, integrated approach to measuring the quality of care for chronic disease and preventive services in the doctors' offices. Three components of a defined measurement set was assessed and consisted of: (1) clinical performance measures; (2) office systems; and (3) patient experience of care. Three Medicare Quality Improvement Organizations (QIOs) participated in the project and included the QIOs for Iowa, California, and New York. The measures focus on chronic conditions and preventive care that are prevalent in the Medicare population and treated in primary care. The DOQ Report and Appendices are available in the Downloads section below. The DOQ project goals were to: - assess quality of care in the doctors' offices
- assess the feasibility of collecting data using a defined quality measurement set
The quality measurement set is made up of three components: - Clinical Performance Measures, include: coronary artery disease, diabetes mellitus, heart failure, hypertension, osteoarthritis and preventive care;
- Patient Experience of Care Survey, include: appointment access, continuity of care, communication, health promotion, interpersonal treatment, office staff, and integration of care; and
- Physician Practice Connections (PPC), which will collect information from doctors' office staff. This tool will evaluate three categories of standards: Clinical Information Systems, Patient Education Support and Care Management.
DOQ Efficiency Measure Final Report This report, available in the Downloads section below, is the final deliverable of the CNA Corporation (CNAC) for the DOQ project and is a culmination of their work on the issue of improving the quality of care for chronic disease treatment and prevention at the level of the individual physician or medical office practice. The report describes the efficiency concept and applies it to various perspectives – those of patients, physicians and CMS. It also depicts how CNAC estimated a model based on the efficiency concept, the results obtained, and some of the implications of the requests for policy and practice.
