The purpose of this site is to store and disseminate technical information related to the development of the Long Term Care Resident Assessment Instrument Version 3.0 of the MDS (Minimum Data Set). The information posted here is intended to inform nursing homes, State agencies, software vendors, professional associations and other Federal agencies of the activities underway to refine and evaluate the MDS.

The Centers for Medicare & Medicaid Services (CMS) would like to inform the long term care industry of a Minimum Data Set, Version 3.0 (MDS 3.0) Special Open Door Forum (ODF), scheduled to be held in the CMS auditorium in Baltimore, Maryland on January 24, 2008 from 1:00 pm to 3:00 pm EST to report on the findings of a 5-year CMS Nursing Home MDS 3.0 Validation Study. CMS plans to implement MDS 3.0 changes nationally on October 1, 2009. Telephone registration is required and can be accessed under Related Links Inside CMS below. As a result of high demand for participation in the Special Minimum Data Set 3.0 Special Open Door Forum, CMS has worked diligently to obtain additional lines to accommodate as many telephone participants as possible. Upon registration to the ODF, you will receive a confirmation e-mail containing further participation information. The deadline for registration is 3:00 PM EST, January 23, 2008. Although onsite registration is now closed, CMS will accommodate all those who previously received onsite confirmation. Participants are encouraged to share a single telephone line. This website will be accessible for downloading beginning January 30, 2007 and will be available for 30-days. CMS plans to evaluate the impact of the MDS 3.0 changes on the resident classification system, Resource Utilization Group (RUG-III), used in the Medicare payment structure. This analysis will be conducted as part of the Staff Time and Resource Intensity Verification (STRIVE) study and the results will be available in late 2008/early 2009. Then, the MDS 3.0 changes can be finalized and implemented nationally on October 1, 2009.

The MDS 2.0 instrument has not had extensive clinical updating since 1995 and based on feedback, it has been designed to improve data assessment, care planning, quality measurement and resource utilization. Proposed Changes are based on extensive written and oral provider feedback, a Town Hall meeting, CMS/Veterans Administration Research Collaborative, technical expert review and data collection in 8-States. The rationale for revising the MDS came from several sources. Nursing home providers were concerned that the MDS was not clinically up to date, valid and relevant. Nursing home staffs complained of a time burden in completing the MDS without clear benefit and some MDS items were described as difficult or unclear. Nursing home residents and their families were concerned that MDS assessment items were completed primarily by staff without their direct input.

MDS 3.0 History

In April 2003, the Centers for Medicare & Medicaid Services (CMS) issued a contract to RAND to evaluate proposed revisions to the Minimum Data Set for Nursing Homes (MDS 3.0). The current MDS, version 2.0 (MDS 2.0) is being revised based on measurement science and on feedback and guidance received from stakeholders, users, and content experts. Starting 2006, the MDS 3.0 draft version is being evaluated on-site in a national sample of community nursing homes in 8 States (New Jersey, Pennsylvania, Georgia, North Carolina, Colorado, Illinois, California and Texas) and in a separate sample of twenty Veteran Administration nursing homes to ensure that the instrument is feasible to collect and improve resident assessment. The reasons for the MDS 3.0 revision are broad:

  • To make the MDS more clinically relevant, while still achieving the federal payment mandates and quality initiatives;
  • To improve ease of use and efficiency;
  • To integrate selected standard scales; and
  • To elicit resident voice by introducing interview questions.

  • Validation Test Draft MDS 3.0): The draft version of the MDS 3.0 dated June 30, 2006, which contains a written introduction to MDS 3.0 and items that are being tested and may be changed after analysis, is also available as a download. It should be noted that many current MDS 2.0 items will not be changed and are not included in this draft. (Working Draft)
