2007 Employer/Union-Only Group Waiver Plan (EGWP) Applications

Below are the Employer/Union-Only Group Waiver Plan (EGWP) application materials for the 2007 contract year. (Please note that based on the comments CMS received after the draft applications were posted in December 2005, CMS has added one additional direct contracting option for employers and unions (Direct Contract Medicare Advantage Organization - Private Fee-For-Service)).

These are the final versions of the applications, and must be completed and received by CMS no later than Monday, March 20, 2006.

New Applicants:

The following applications are to be used for new applicants only:

• Employer/Union Direct Contract Prescription Drug Plan (PDP) (one application)
• Employer/Union Direct Contract Medicare Advantage Organization (MAO) (Private Fee-For-Service only) (two applications – MA-PFFS & MA-PD)
• PDPs, MAOs and Cost Plan Sponsors seeking to offer new employer/union-only group plans (one EGWP application for each entity type)

Service Area Expansions (SAE):

The remaining application is to be used for existing 2006 EGWPs that wish to expand their service area (not needed for existing Employer/Union Direct Contractors).
