Registration Instructions	Non-committee members must register via email or regular mail to Michael Keane if they want to attend the meetings. Registrations must be received by 5pm, February 18th, 2005. Addresses are as follows: sean.dalenberg@cms.hhs.gov or Sean Dalenberg, Mailstop C5-08-27, 7500 Security Boulevard, Baltimore MD 21244. Public comment will be allowed if time permits. An agenda identifying subjects to be discussed will be posted on this website, January, 2005. Each registrant must provide the following information: Name, telephone number, email address (if applicable), company name and address. Because of staff and resource limitations, we cannot accept registration/comment by facsimile (FAX) transmissions.
Special Instructions	Persons attending the PAOC public meeting with hearing or visual impairments, special requirements, or a condition that requires special assistance or accommodation, must provide this information upon registering for the meeting.
