For People with Medicare

MEDICARE HEALTH SUPPORT offers individualized services that can help people:

  • Keep track of their medical treatments;
  • Get support in following their doctor's plan of care; and
  • Learn more about their chronic conditions and medications

MEDICARE HEALTH SUPPORT does not take the place of a doctor. Instead, it helps people understand and follow their doctor's advice. The MEDICARE HEALTH SUPPORT program may include calls or visits from a nurse, educational materials, in-home health monitoring equipment if needed, and more.

Congress created this program to help people with Medicare stay healthy and reduce medical costs. Participation in MEDICARE HEALTH SUPPORT is a choice. If you were invited, you can use the program as long as you like. No one has to participate. It is free of charge to participants.

MEDICARE HEALTH SUPPORT programs are operational in five areas at this time.

  1. Western Pennsylvania
  2. Washington, D.C. & Maryland
  3. Chicago, Illinois
  4. Central Florida
  5. Select Tennessee counties
